Neuropsychological Evaluation

Developmental and Educational History:

Elementary school: She denied any history of academic problems during elementary school except to note that she was a "terrible speller" and she experienced some difficulties with punctuation.

BEHAVIOR OBSERVATIONS/MENTAL STATUS:
She demonstrated no significant disturbance in content of thought. However, she appeared to be distracted easily by noise or pictures on the wall during the testing.

SUMMARY:
Ms. Hartman is a 27-year-old single, female who is currently enrolled in the master' program in landscape architecture at the University of Illinois. She complained of longstanding and pervasive difficulties in attention/concentration, task completion, and procrastination.

Furthermore, she has demonstrated much perseverance despite many of her difficulties and this character trait will continue to serve him well as she works through her current difficulties.
